The question we set out to answer:
Rational 360 partnered with PeakMetrics to understand and quantify how trusted industry voices in healthcare are among Beltway voices, including policymakers, congressional staff, political reporters, think tanks, advocacy organizations, and other D.C.-based influencers.
Together, we set out to move beyond surface-level sentiment and answer a more nuanced question: How much institutional trust exists within the federal policy ecosystem, and how is that trust expressed?
Leveraging PeakMetrics’ Smart Categories to answer the question:
Trust is intangible and difficult to measure using traditional keyword searches. While we can search for explicit words like “trust” or “confidence,” those terms capture only a small fraction of how people actually communicate trust or distrust in practice.
PeakMetrics’ Smart Categories allowed us to move beyond rigid keyword Boolean queries and instead build AI prompts around emotional framing and thematic signals that determine the level of trust that a Beltway influencer has. The model evaluates context, tone, and meaning across posts. This approach enabled us to capture a broader spectrum of trust signals, including implicit expressions of credibility, skepticism, confidence, alignment, or institutional faith, that would have been nearly impossible to isolate manually.
In addition, PeakMetrics helped us build a Smart Category to identify Beltway voices using author-level data, including author profile info, follower counts, and publisher details. By combining the Smart Category for Beltway voices with the Smart Category for public trust, we ensured that we evaluated trust specifically within D.C. policy influencers, rather than the general public.
Together, this dual-layer approach, identifying the right voices and accurately interpreting how they express trust, allowed us to measure a qualitative concept at a quantitative scale.
The Analysis: How Much Do Beltway Voices Trust the Healthcare Industry?

The results show a complicated trust landscape, one shaped by ongoing policy debates, drug pricing concerns, and the industry’s role in healthcare innovation.

A Trust Deficit in the Conversation
The largest share of mentions reflected skepticism toward industry stakeholders.
However, these positive perspectives were relatively rare compared with the broader skepticism present in the conversation.
- 25.7% of posts expressed deep distrust, signaling strong concerns about corporate motives, pricing practices, and influence over public policy.
- 27.2% reflected low trust, often referencing transparency issues or perceived political influence.
Combined, this means over half of the conversation (52.9%) leaned clearly distrustful, highlighting the uphill battle the industry faces in shaping public perception among influential Beltway voices.
A Significant Middle Ground
Despite the skepticism, the conversation wasn’t entirely negative.
- 25.9% of mentions fell into a moderate trust category. These posts acknowledged the industry's contributions to innovation, but still raised concerns about access, or regulation.
- 15.3% expressed mixed or conditional trust, where trust depended on specific policies, reforms, or corporate actions.
Together, these categories reveal a large segment of voices that are not firmly opposed to industry, but are watching closely and evaluating its behavior.
The Bottom Line
Among Beltway voices, there is an opportunity to expand trust among healthcare industry voices. While skepticism dominates the conversation, a substantial portion of commentators fall into a middle ground, acknowledging the industry's importance while calling for greater accountability and transparency.
